Phone number: enter without any gaps eg. for Spanish numbers: 697XXXXXX, for foreign numbers put the country code eg: +44XXXXXXXXX
Insurance: select Private if your insurer does not appear in the list
E-mail: make sure your email is entered correctly. You will receive an automated confirmation and further information to this address.
Date Selection: make sure the calender is for the month that you want an appointment (days either side of the month that are showing are not bookable).
